PrintDocument does not print the whole document

Printing the attached document with the code below prints only one empty page with header and footer. If the two textboxes on the left border of the first page were removed the whole document will be printed but not in the correct format (converting to pdf or image have the same rendering problems). Please provide a fix.

PrintDialog dialog = new PrintDialog();
document.PrintDialog = dialog;
dialog.AllowPrintToFile = true;
dialog.AllowCurrentPage = true;
dialog.AllowSomePages = true;
dialog.UseEXDialog = true;
if (dialog.ShowDialog() == DialogResult.OK)
{
document.PrintDocument.Print();
}
