PDF to XLS conversion issue

Dear Sir/Madam,

When converting PDF document (which was originaly in XLS) to XLS is breaking into multiple sheets instead of single sheet. Also, multiple columns are shown for single column of PDF document. The PDF document is attached for your reference.

Kindly suggest to convert with proper formatting.
