I'm hitting some issues when working with a large PDF in a thread and before I go too much deeper into the issues, I wanted to check that Spire.Pdf is thread safe?

Thanks for your inquiry.
If you process only one PDF in one thread, then the application is indeed thread safe. But if you use multiple threads to operate the same file, then it may cause an exception due to multiple threads accessing the same resource at the same time. So please make sure you are not using multiple threads to operate the same file.
